S6-Scan™ Formation Diagnostic
A structural readiness assessment for founder-operators and hub leaders. This intake determines whether your hub is ready for formation diagnosis—not strategy consulting, not a pitch, but a clear-eyed look at what's actually true about your system right now.
Purpose
What This Intake Actually Does
This is not a sales form. It's not a strategy consultation request. This intake begins the diagnostic from ground truth—what's real about your hub, not what sounds good on paper.
Short, direct answers beat polished ones. We're looking for signal, not spin. The questions probe formation readiness: traction, continuity, effort distribution, and structural integrity. Answer plainly and you'll get clarity on whether the S6-Scan fits what you're actually building.
Honest Assessment
We start from what's true, not what's aspirational. Clarity begins with candor.

Section One
Define the Hub Being Assessed
Three Core Questions
What is the hub you are building? One clear sentence. Person, business, organization, platform, idea, or mission. Name the node we're diagnosing.
What problem does it exist to solve? The specific gap, tension, or need that creates demand for what you're building.
What does this become if it succeeds? Program, platform, institution, movement, or category. Name the future state—the form it takes at full expression.
Formation Signals: Timing and Traction
01
Why Now?
What changed in the world, market, or environment that opens the window? Formation happens when conditions align. What shifted to make this possible or necessary now?
02
Where Does This Already Have Traction?
Communities, industries, platforms, partnerships, or geographies. Where is it already taking hold without massive force? Traction reveals natural fit.
Effort and Continuity Diagnosis
Revealing Questions
What currently requires the most effort to keep moving? Funding, attention, partnerships, belief, operations, adoption—be specific. Where are you pushing hardest? This reveals structural friction.
If you stopped pushing for 30 days, what would stop working? This exposes continuity. What has momentum independent of your constant input? What collapses without you? Answer plainly—this is diagnostic, not judgment.
These questions map where energy goes and what sustains itself. Formation readiness often shows up in the gap between the two.
Hub Classification and Stage
Hub Type
For-profit venture, nonprofit or mission organization, hybrid social enterprise, institutional initiative, or personal platform identity hub.
Current Stage
Concept and early formation, building and launching, operating and growing, scaling and institutionalizing, or transitioning and re-forming.
Financial Readiness
Standard diagnostic fee acceptable, possible depending on fit, nonprofit requesting reduced or sponsored rate, or unsure and needing clarification first.
Clarity Check
Fit and Intent Assessment
Two Essential Questions
Why are you requesting an S6-Scan now? What prompted you to seek formation clarity rather than more strategy?
Most founders have access to strategy advice. Plenty of consultants will tell you what to build or how to position. But formation diagnosis is different—it assesses structural readiness, continuity, and whether the hub can sustain itself beyond founder will.
If you're here because you want validation for a plan, this isn't the right tool. If you're here because something feels misaligned and you need to see the structure clearly, then we're likely in the right territory.
Commitment Signal and Next Steps
Understanding Required
The S6-Scan is a paid structural diagnostic. It is not consulting, not coaching, and not a pitch. You're paying for clear-eyed assessment of formation readiness.
Review Process
Submission does not guarantee acceptance. Fit is reviewed before scheduling. We assess whether the diagnostic matches what you're actually building and where you are in formation.
After Submission
Qualified requests will be invited to schedule an S6-Scan Briefing call. This call confirms fit and sets up the full diagnostic engagement.
Submit Your S6-Scan Intake Request
Answer the questions directly and specifically. Short, honest responses give us what we need to determine fit. After submission, qualified requests move to a briefing call where we confirm whether the S6-Scan matches your hub's formation stage and readiness.
